Self-management programs provide you with the tools to manage your diabetes and take control of your health and well-being. Both free and paid programs (covered by insurance) are offered.
Free Diabetes Self-Management Programs
If You Have Pre-Diabetes
If you have higher than normal blood sugar (pre-diabetes), simple lifestyle changes can greatly reduce your risk of developing type 2 diabetes. The free National Diabetes Prevention Program consists of 16 weekly one-hour class sessions to help you manage your blood sugar and prevent diabetes, with followup support after classes end.
If You Have Diabetes
The Diabetes Self-Management Program is a series of free workshops based on the Chronic Disease Self-Management Program developed at Stanford University. Participants meet for several hours each week for six weeks in highly participative workshops where mutual support and success build their ability to manage their health and maintain active and fulfilling lives.
Doctor-Referred Management Program
The Freedom Diabetes Self-Management Education and Support program guides you in the essentials of eating right, exercising, and monitoring your health. The Freedom team works with you and your doctor to create your self-management plan and provide support after classes end. A doctor or health clinic can see whether you qualify and refer you to this program. The cost of these classes is covered by Medicaid, Medicare, and some private insurers.
