As of January 2026, the Healthy Moms/Healthy Babies (HMHB) program is not accepting new referrals and is not enrolling new participants.
We appreciate your interest in the program and we encourage providers and community partners to continue using other available maternal and infant support services in their area. Updates regarding referral and enrollment status will be shared as they become available.
For Medicaid-enrolled patients, please refer individuals to their Coordinated Care Organization (CCO) for case management services.
