Incident reporting for licensed healthcare providers only.
Licensed healthcare providers, are now required to use the online form below for all incident reporting required for your facility type. This does not replace the Federal and State mandated timeframe to call the Hotline as a first notice, but does replace both e-mailed and faxed reports currently in use.
Resuming a Partial Submission
Be sure to record the return code provided upon partial submission of an incident in order to resume the report later. MSDH staff are not able to provide this code to you.
