Documenting and Reporting Vaccine Administration
All providers enrolled in the COVID-19 Program are required to document the administration of vaccine in MIIX within 24 hours.
If your clinic is not set up to report vaccine administrations in MIIX, please use the MIIX User Agreement to begin the process.
Reporting Adverse Reactions (VAERS)
The preferred way to submit an adverse reaction report to the CDC's Vaccine Adverse Event Reporting System (VAERS) is with their online form.
