
The MSDH Office of Preventive health offers to the public the Stanford University Chronic Disease Self-Management program and the Diabetes Self-Management Program — a free six-week step-by-step workshops for adults who are living with or caring for someone with a chronic condition or disability.
You don't have to stop doing all the things you love in order to enjoy a healthier life!
What is the Motivated to Live a Better Life program?
A free self-management program for any one living with a long-term health condition such as:
- arthritis
- asthma
- cancer
- diabetes
- depression
- heart disease
- high blood pressure
- chronic pain

How You'll Benefit
By taking part in Motivated to Live a Better Life you will learn ways to cope and manage your health by:

Program Details
Participants meet 2½ hours each week for six weeks. Workshops are held in community settings such as churches, community centers, libraries, hospitals and other locations throughout the state. There is no charge to attend a workshop.
