COVID-19 Vaccination Provider Enrollment, Vaccine Orders
If your facility plans to administer COVID-19 vaccine, you must enroll as a COVID-19 provider. For healthcare personnel or a healthcare facility to receive and administer vaccine priority groups, including healthcare staff, you must be enrolled as a COVID-19 vaccine provider.
Individual healthcare providers and personnel do not have to be enrolled as COVID-19 vaccine providers to be vaccinated against COVID-19.
Every vaccine provider should have an emergency plan. If you don’t have one developed for your clinic, please use our COVID-19 Vaccine Accountability and Management Plan as your emergency planning resource.
MSDH is offering funding from the CDC to ensure greater equity and access to COVID-19 vaccine for those disproportionately affected by COVID-19. MSDH needs the assistance of enrolled immunization providers to administer COVID-19 vaccinations within their communities. Populations that have been disproportionately affected by COVID-19 include, but are not limited to:
Providers may now make orders on an as-needed basis. The chart below provides the minimum order quantities for each vaccine formulary currently available.
Moderna Bivalent
6 months – 5 years
Minimum order: 20
Only to be administered to individuals ages 6 months through 5 years who have received 2 previous doses of the Moderna Monovalent Vaccine or to certain immunocompromised individuals ages 6 months through 5 years.
Moderna Bivalent
6 months and older
Minimum order: 100
Pfizer Bivalent
6 months – 4 years
Special order
Pfizer Bivalent 6m–4y must be requested through this online request form.
Pfizer Bivalent
5 – 11 years
Minimum order: 100
Pfizer Bivalent
12 years and older
Minimum order: SD 50 / MD 180
Novavax Monovalent
12 years and older
Minimum order: 100
Vaccine transfers are approved on a case-by-case basis. Vaccine should be ordered directly through MIIX unless there are extenuating circumstances.

Janssen doses are no longer available for order.
To receive continuing education (CE) or to print a certificate, visit Training and Continuing Education Online (TCEO) and follow these nine simple steps for each course.
All providers enrolled in the COVID-19 Program are required to document the administration of publicly funded vaccine in MIIX and to maintain the Vaccine Administration Record (VAR) for each patient within 24 hours of administration.
If your clinic is not set up to report vaccine administrations in MIIX, please use the MIIX User Agreement to begin the process.
The preferred way to submit an adverse reaction report to the CDC's Vaccine Adverse Event Reporting System (VAERS) is with their online form.
