O'Charley's Southaven Complainant and her husband went to O'Charley's on July 2, 2011. The complainant's husband ordered ice tea and there was a knat on the side of the glass. The complainant and a friend went to O'Charley's on July 9, 2011 and had a strawberry lemonade. The first drink was fine and the second drink had a fly.

Ate time of arrival , I spoke with Randy Carmody with regards to the complaint. Per Mr. Carmody, he spoke with a representative from Ecolab with regards to obtaining a fly treatment program on July 1, 2011. Ecolab treated the facility on 6/21/11 for mice and cockroaches per invoice. During walk thru, a small number of flies were observed. The facility must obtain a fly treatment program and provide documentation to my office within 3 days. Knats were not observed at time of inspection. Per documentation received at the environmental office, the facilty the facility was treated for flies on July 15, 2011.
