Caller at around 1pm on Tuesday 10/13/2020 Signs and symptoms started around 4 or 5pm Tuesday afternoon. First symptom was nausea and was followed by abdominal cramping, diarrhea and vomiting Caller stated she went to the emergency but was not asked to submit a stool sample. Caller states ER Doctor told her she had "food poisoning" Per epi.... Received a call this morning regarding "foodborne illness" after eating at Taco Bell. Caller states she consumed a #5 (Nacho BellGrande), but did not eat the taco that came with it. Caller states she still has the taco and container that the meal came in and requested that we test it for "E.coli" I informed the caller that at this time we would not test the food she had, but we would complete an inspection at the facility.

Visited this food facility due to a requested complaint. Spoke with mgr, Valerie Jordan, about the nature of this complaint. McClain co delivered this product to Taco Bell. Tyson food distributor in Springdale, Ar also Housed this product. The meat product comes in pre-cooked and is stored in the walk-in cooler. Cooler are functioning properly. Pre cooked beef is then reheated in the theremolizer to 165 or higher then hot held. Reheating takes approximately 30mins. If any is left at closing time, it is put in an ice bath and lowered to 32-33f. Then set inside the walk-in cooler. The next day the product is taken through the same reheating process and used up. Shredded lettuce comes in cold as well and placed in the cooler then put on cold line when ready to use. Cheese comes in pre-packaged and is stored on the pantry area shelf. Cheese is heating also in the thermolizer the same as meat but for 16-30 mins to at less 165f. No persons were sick during the time of customer purchase food per mgr. Manager states that she observes all employees for sickness and unusual behavior. 3serve safe certification were observed in the facility and proper sanitizing of dish ware was in place. Sanitizing buckets were tested with proper ppm(200-220ppm for quat).
